ABSTRACT

Objective To establish a blast injury experimental model using a shock tube at lateral lying position of C57BL/6 mice, investigate biomechanical responses of macrophages/microglia cells in the heart, lung and brain tissues to mechanical damage by shock wave within 24 hours. Methods Shock tube was employed to generate a shock wave to C57BL/6 mice. Firstly, the weight changes of mice were measured at different time points after the shock. Then the cardiac, pulmonary and whole brain tissue samples were dissected after anesthesia. Pathological sections were stained with HE staining to detect structural damage; the TUNEL staining method was used to mark and count the proportion of dead cells in each tissue. Microglial cells were labeled with fluorescent antibody, while responses and changes of macrophages/microglia after shock loading were analyzed. Results The shock tube exerted 179 kPa overpressure shock wave upon sideway of the mouse, and lethal rate of the mouse was 3.33%. Compared with normal control group, the mice in experimental group had a significant weight loss within 24 hours after loading shock. Pathological sections showed rupture of lung tissues after shock, accompanied by alveolar protein deposition, pulmonary bulla and other diseases. Fluorescence staining showed that lung tissue was recruited and activated in a large amount within 24 hours. The proportion of dead cells cleared rebounded to normal level within 24 hours. The heart was highly tolerant to shock, and macrophages appeared near the large blood vessels. The brain showed unilateral aggregation of microglia due to the impact posture, mainly due to prolonged inflammation and a higher proportion of dead cells at the junction of gray and white matter. Conclusions A blast shock model at lateral lying position of the mouse was established. Within 24 hours, macrophages/microglia were recruited quickly to the injury site after being impacted, which mediated strong immune stress, and might participate in the immune response to trigger a second long-term inflammatory injury. The results of the study provide experimental basis for the evaluation of primary impact injury, such as dose-effect relationship and tissue damage difference.

ABSTRACT

<p><b>OBJECTIVE</b>The aim of this study was to assess the feasibility and safety of laparoscopic nerve plane-sparing radical hysterectomy (NPSRH) and compare with that of open NPSRH.</p><p><b>METHODS</b>One hundred and thirty-four patients with FIGO stage Ib1-IIa2 cervical cancer were enrolled in the study. Thirty-three patients underwent laparoscopic NPSRH. During the operation, the pelvic autonomic nerve plane which is directly underneath the ureter was integrally preserved by dissecting the pelvic spaces laparoscopically. The vessels around the nerve plane were controlled by Hem-o-lok polymer clips. One hundred and one patients underwent open NPSRH without special instruments. The clinical, pathological and surgery-related parameters were compared between the two groups. Moreover, postoperative short-term bladder function of these patients was also analyzed.</p><p><b>RESULTS</b>There was no significant difference between the laparoscopic group and open group in terms of age, body mass index, previous surgery, FIGO stage, pathologic type, etc. (P > 0.05). The mean duration of surgery in the laparoscopic group was significantly longer [(303.8 ± 67.5) min vs. (272.4 ± 57.5) min] (P < 0.01). But, the laparoscopic group had less blood loss [177.0 ml vs. 474.5 ml, P < 0.01] and blood transfusion rate [ 6.1% (2/33 cases) vs. 49.5% (50/101 cases), P < 0.001]. There was no significant difference regarding the proportion of patients who firstly passed the post-void residual urine volume (PVR) test (P > 0.05). The median time of catheterization between the two groups were also comparable (P > 0.05). However, the postoperative hospital stay was significantly shorter in the laparoscopic group [median postoperative hospital stay 9.2 days vs. 11.0 days, P < 0.001].</p><p><b>CONCLUSIONS</b>Laparoscopic NPSRH is feasible. It seems to be comparable with open NPSRH in terms of preserving pelvic nerve function, but is more favorable in terms of blood loss and postoperative recovery.</p>

ABSTRACT

Objective To study the correlation between serum adiponectin along with insulin resistanee and vascular dilation function in patients with different rates of glucose metabolism.Method A total of 83 patients aged 50-65 years,including 17 with normal glucose tolerance(NGT),22 with impaired fasting glucose (IFG),23 with impaired glucose tolerance(IGT)and 21 with diabetes(DM)were enrolled. Serum adiponectin was measured,and homeostasis model assessment for insulin resistance index (HOMA-IR)and B cell function index were used to evaluate the function of insulin secretion before and during oral glucose tolerance test(OGTr).The brachial artery responses to flow-mediated endothelium-dependent vascular dilation(EDVD)and nitroglycerin-mediated vascular dilation (EIVD) were evaluated by using the vascular ultrasound with high resolution.Results The serum adiponectin and EDVD decreased that the degrees of reduction from slightness to greatness were in turn from NGT,IFG,IGT to DM,whereas,the insulin resistance index (HOMA-IR) increased just in reverse order.Adiponectin was negatively associated with HOMA-IR(r=-0.353,P0.05).In a multiple regression analysis with a stepwise manner to predict adiponectin concentration and EDVD.HOMA-IR was supposed to predict adiponectin concentration.Meanwhile APN but not HOMA-IR was used to predict EDVD(P<0.05).Condusions The decrease in serum adiponectin,endothelial dysfunction and insulin resistance can be observed in the early stage of impaired glucose metabolism. Serum adiponectin concentration appears to be associated with vascular function and insulin resistance.The association between serum adiponectin concentration and vascular function seems to be independence from its link with insulin resistance index.
